When you accept your first case you’ll have to provide to you employer/contractor the following information:

3. Malpractice insurance ($1,000,000.00). If you do not have malpractice insurance, and do not know where to get it, please call us for information. Annual policies usually cost under $100.
4. Copies of your passport/green card and driving license.
5. Two professional references.
6. Annual physical exam.
